Computer
downlink allows Stoddard County Public Health Center access to vital
records at Missouri Department of Health in Jefferson City,
Missouri.
Certified copies of Birth Certificates can be printed at our office
while you wait. Certificates are available for any birth
occurring
in the State of Missouri after 1920.
Certified copies of Death Certificates can be printed for any person
whose date of death is after 1980
Birth Certificates
are $15.00 each. Death Certificates are $13.00 for the first
one, and $10.00 for each additional copy.
The Health
Department can only accept cash or money orders.

To request a death certificate, print
the following form, complete, and bring or mail to the Health
Department along with the money order per certificate fee.
Certificates are available immediately if you present the form in
person. Allow one to two weeks for certificates requested by mail.
